You can't answer this question directly since it involves sensitive information, but that information aside, you have to imagine that Darth Vader was famous throughout the galaxy. Even people on Tatooine would have to have heard the name.
It's definetely not a continuity problem.

Darth Vader is the right hand man to the ruler of the galaxy. I'm sure that his name is very well known throughout the galaxy.

Well, he told Luke that his Jedi pupil Darth Vader killed Anakin. So obviously he knows the name.

It's really simple. At that point, the guy's real name was "Darth Vader". He wasn't Luke's father at the time the first film was shot (there's evidence that Lucas didn't come up with--or was it take under advisement?--that idea until 1978).
Even wonder why everyone calls Vader "Lord" if "Darth" is supposed to be a title or rank? That's because it was originally supposed to be his *real* first name.
That's like saying, "Greetings, Captain Mr. Kirk!".

Darth, yes is a title, but it also was Darth Bane's first name and as he made the rule of two, he also passed the Darth name down from Sith Lord to Sith Lord.

Darth is his title. So Obi-Wan, knowing that Anakin Skywalker is no more, would call him Darth...
...just as he called Chancellor Palpatine by Chancellor, and Senator Amidala by Senator.

There's no way Obi-Wan wants to think of Darth Vader, Lord of the Sith, as Anakin. He avoids this thought as much as possible in the OT, though it's always there in the subtext. It's just not something that is very pleasant for Obi-Wan for a variety of reasons.

It's a title.
Like people calling an admiral "admiral", or a captian "captian".

When Darth Vader says: "When we last met, I was but the learner, now *I* am the Master.!"
Yeah, like Ben would have asked him:
Obi-Wan: "Now, Anakin... I;m sorry... I know that the name "Anakin Skywalker" has no meaning for you, since... sorry about that... 'volcanoes", right? And "Darth" is your Sith title, and "Vader" is your Sith name... but did your Master, Sidious ever give you have a "first" name, like "Rick," or "Joe," or ,"Mike"?"
Vader: "Yeah, my Master says first name is "Terry"."
Obi-Wan: "You're only a Master of Evil, Terry...."
LOL

I always took Darth to be the ancient Sith word for Lord. Kinda how we had Ceasar, Kaiser, and Czar. That Obi called Vader "Darth," to me, meant he thought Darth was his first name because Obi had never encountered a Sith he knew by name before and didn't know Darth was a title.
All this will just prove how jarjarish I am, come May 19.

Well, I figure at the time, "Darth" really was the character's name, I still think that's the case now. If his name were, I dunno, Mitch Vader, you'd hear Ben saying "Only a master of evil, Mitch!"
That and Ben seems to be rather condescending to Vader, belitting his former student by downplaying his achievements through the Dark Side. He's not "Lord Vader" to Obi-wan, he's still that punk he beat twenty years ago. So calling him "Darth," by first name, instead of a title is an added little slap saying "You're not as great as you think you are. You're still Darth to me, Darth."
