Great work, ardavenport!  Although this questionnaire was (meant to be) somewhat silly, you managed to capture Qui-Gon's personality and relationship with Obi-Wan excellently using just a series of numbers! I loved Qui-Gon's answers to the open-ended questions best. 
I would be very interested to see responses by our other favourite Jedi. Maybe that will help us figure out who these surveys originated from...

This was fantastic! I loved every minute of it. Especially where he agreed that he worked well with others but disagreed that others worked well with him!
If you don't think a survey with Obi-Wan would work, perhaps a vignette then? Qui-Gon could have printed out his results and left them lying around in their apartment. Obi-Wan could find them and we'd get to see his reaction and thoughts to Qui-Gon's answers.

Ah, workplace surveys. One of my favorite things to complain about.
I love that you're usuallly assured of confidentiality, then they ask enough classification questions to identify you. Sure, supposedly the answers are aggregated to prevent that, but still...there's no doubt this is Qui-Gon. Not that he cares if anyone knows it.
I wonder how old Qui-Gon (and thus Obi-Wan) are in this piece. The disharmony in their relationship and Obi's disobedience initially made me think it was early in their partnership. But it could be as Obi-Wan matures and is asserting his own views a bit more.
(5.) What activities most hinder your growth in the Force?
I am engaged in one of them right now.
(6.) If there are any other issues that have not been covered by this survey, please list them here.
I believe that we need fewer surveys.
I agree, Qui-Gon, I agree.
This was hilarious! Good job.
