It was never clear to me just
why
Palpatine's throne room contained a shaft, down which a person could easily fall. He doesn't seem terribly busy; virtually all we see him doing during RotJ is sitting in his chair and looking out the window while he waits for Vader to find Luke. You'd think, at some point, it would have crossed his mind that he could ask someone to do something about the huge, potentially deadly shaft in the throne room where he planned to confront Luke and goad him into engaging in a fight.
He could have said, "Hey, I want you workers to move all of my stuff into another room... one where there's not a ventilation shaft I can fall into," or perhaps "Maintenance, could you please put a grate on top of that huge, deadly, gaping hole in the middle of the room where I spend all of my time?" It seems like those are things he could probably convince people to do easily.
I think that's why he's shouting as he falls down after Vader throws him in. He's angry at himself for being so stupid as not to realize that he should have built his throne room someplace where there
wasn't
a bottomless pit.
Just a thought.

Well I never understood why on Naboo there is a giant shaft to the planet's core with energy gates that open and close at certain intervals. It's just a lame plot device to keep Obi-Wan and Qui-Gon separated. I think the original design for the Imperial Palace on Coruscant involved lava in the Throne Room. Apparently Palpatine has a weird aesthetic preference.

Maybe because he never expected to be killed in his own throne room?

Maybe I'm making this up, but I think I remember hearing his throne room on the DSII was something thrown together last-minute? Can anyone back me up here? Like, the workers didn't know in advance he was coming and didn't have any space ready for him, so they had to appropriate whatever space was available, and it just happened to be some control chamber with a shaft. Something along those lines.
That said, I'm kinda surprised he didn't fire on sight every contractor who thought it was a good idea to put his swivelly chair over a reactor. (You guys suck at designing corner offices!!)

Because late at night, when no one's around, Emperor Palpatine glides
up to the shaft, leans over the rail, and yells, "ECHO!"
Echo... echo... echo..

I would agree that it's probably part of the aesthetic Palaptine desired. The Dark side is all about fear, and what better way to get the juices flowing when you're staring down an endless shaft?

Cause how pimp would it be to have a tube to the reactor core in your thrown room. If the Emp's Death Star was on MTV's Cribs, all the homies would be like WOAH!
