General Grievous attacks in Saga Edition

I'm confused.... 
His attack bonus for lightsabers is +21.
When when he uses 2 lightsabers it is at +19 and +19 (because of his Dual Weapon Mastery I & II).
Then when using 2 lightsabers he can use the Double Attack feat (which grants 1 extra attack, with a -5 to all attacks) giving him +14 and +14 and +14 for 2 lightsabers with Double Attack.
This I get... but:
+11 and +11 and +11 with Double Attack and +11 with Triple Attack
I do not get.
Could someone explain the mechanics of this please. It would be a great help.

i'd be surprised if it's not errata, I was having a read through a half hour ago and noticed Yoda's attack bonus with triple attack is +29/+29/+29 while his single attack is +29 and double attack +24/+24
