Here's mine for the Jedi:
Mari-Elan Nora
Mari-Elan nearly missed becoming a Jedi. Born on the grim world of Necropolis, she went undiscovered by the Jedi Order until the age of seven. She was one of several Force-sensitives discovered by the maverick Jedi Master Djinn Altis. Like his other students, she was trained alongside multiple padawans, but she never truely believed in his dissidant philosophy and missed the authority that came with affiliation with the Temple on Coruscant. She left Altis at the age of twelve and petitioned the Jedi Council to join the Order, a request initially denied. Nora's skill and discipline could not be denied, however, and she was finally accepted as a padawan.
After passing the trials, Mari-Elan chose not to take a padawan herself, and instead accepted a teaching position at the Jedi training center on Kamparas. When the Clone Wars broke out, Nora recognized the need to complement the Jedi's shrinking ranks. She volunteered to coordinate the military actions of the Order with factions sympathetic to the Jedi cause, bringing some legitimate athority to what would otherwise be considered rogue groups. She brought her master Altis and his students back into the fold, and also dealt with the Iron Knights, Freedom's Sons, and the Antarian Rangers.
Nora became friends with Obi-Wan Kenobi during the Outer Rim Seiges, and participated in the Battle of Belderone. She attended his briefings on the seiges in order to bring up-to-date information to the group of allies she had assembled. It proved to be the last time she saw Kenobi. While attending a meeting with the Jedi allies on Kamparas, the training center was destroyed from orbit during Order 66.
I wish I'd submitted an entry for the Wol Cabbashite--I love Quest's, but am I the only one who thinks that the Jedi might have room for an intelligent species of immobile slugs? Or that one could attain mastery over a Force that is present in *all* living things? I don't think the Trill/Yeerk-type parasite/symbiote thing is needed here.

Well, the thing is we know they
are
parasites, or at least evolved from parasites.
However, I couldn't see a parasitical creature becoming a Jedi (no more than an Anzati who actively drank soup could remain a Jedi), so I went with the symbiote approach that had a connection to their ecosystem to make it less sinister and more in balance/harmony with it's surroundings. Kinda ties into the whole midichlorian aspect of things too.
In other words, I didn't want a Goa'uld or a Tok'ra. It's been too long since I've seen DS9 to remember what the Trills were like.

Damn, it seems that no one was able to sneak in the name of Lt. Oicuun's Venator Star Destroyer in his entry. I like the fact that he's from Humbarine; ever since the ROTS:ICS mentioned that the world was depopulated by Grievous, I've been seeing more and more references to it.
