Somebody did mention HIM's cover of Wicked Game. That was a great cover. Another cover I really love is Rasputina's Wish You Were Here. That is the only Pink Floyd cover (that I've come across) that I really enjoyed.

An instance of Over the Rhine performing "Hallelujah" is available via YouTube.
The sloppy enunciation, that whiskeywatered drawl, combined with Karin Bergquist's education as a classical vocalist (evidence of which can be heard circa 3:37) is disarming. I think this the most bruising performance of the song, by any artist. I appreciate the little emphases at non-obvious places in the lyric.
It must be said, in Over the Rhine's favour, that their covers are generally more inspired than this take on a previous rendition. The effort still ranks "best," however.

Why does the sun shine? by They Might Be Giants is a cover off of some crappy kids album iirc. And TMBG's version is freaking fantastic.

Metallica has great covers with "Whiskey in the Jar" and "Turn the Page".
Hendrix's "All Along the Watchtower"
Cash's "Hurt" has been mentioned often and I agree.
Then there are numerous great techno/dance remixes that have been done but don't know if that fits in with the topic any more.
