A reminder from the lazy indexer

Seeing as how we have some new Indexers, I thought it would be a good time to remind everyone that having the following information in the first post of a thread will a) increase the chances of it being included in a story index, b) quite possibly attract more readers, and c) make the various indexers' jobs a lot simpler. Please note that you don't have to fill in all the blanks (I usually can't think of any keywords and just put "vignette" for a genre).
