The kid's actually just signalling to one of the directors (or whoever) off-screen that he has to go to the bathroom. Nothing more to it than that really.

For the record, no I do not particularly think a great deal of the Back to the Future series, nor did I find the third installment in particular attractive. Not that I've found anything terrible about them, mind you. I'm just indifferent. But what is all this excitement even about? Why are we zooming in on a child beckoning someone?

So it was kind of a twofer.

 

Why would he do that during a take when he knows he's being filmed and it's going to ruin the take? If he's expecting someone (the director or anyone off camera) to notice it, then he also knows that he's going to make that take unusable so why not just SAY something to ruin the take instead of making some sly motion that no one noticed? Why wouldn't he just ask a director to use the bathroom between takes? If that was his intention AND no one noticed it, then why wouldn't he say "Didn't you see me pointing at my junk??! I have to pee!" at which point someone WOULD have noticed it and they'd have used a different take. If the kid actually had to use the bathroom, every possible outcome of the situation would lead to that take not making it into the movie. The only way it could have made it in is if the kid's intention was for no one to notice him doing it.
