Kashyyyk. When we first see the beach, as the camera passes around the Wookiees, the 41st Elite Clone Troopers and the Scout Troopers. It's very brief. Blink and you'll miss her.

No kidding. The first time I saw ROTS I didn't see her.

Lucas originally had Ki-adi and Plo Koon die on Utapau, but changed it to having them on different planets. Same with all the Jedi, 1 or 2 per planet, and not 4-5 like originally intended. Luminara is shown leading the ground defenses, but is not shown dying. In the script, it says several clones speaking with her pull out pistols and shoot her down. At the same time Quinlan Vos is standing on a turbo tank and is blown up by a cannon. Vos was cut, and it can be reasoned that A) the point of order 66 was proved with 6 jedi deaths and needed no more, B) Obi-wan said Vos was on Boz Pity and it makes little sense for him to show up on Kashyyyk, and C) another Jedi on Kashyyyk was not necessary as there were already 2. Same with Bariss Offee. Shw was supposed to be killed on Felucia, with Aayla, but Aayla's death was enough for that particular sequence.

Watch the second shot of Kashyyyk carefully, when the camera is following up the beach. She's directing a bunch of Wookiees to do something.

Lucas never filmed Quinlan Vos. The explaination that I've heard is that time ran out of Lucas and he didn't put a big rush to film Vos, like he did with other parts of the film.
