Failure to signal was going to be my guess. Really shocked the phone one is there being this survey was done in 1978z

 

Dad had a phone complete with curlycue cord in his government-issued car circa 1963 and used to phone Mom on his way home from work to say when he'd arrive and what he'd had for lunch so she wouldn't fix it for dinner. Way to go, Dad!
