This is the list from Clone Wars/Rebels:
    1. The Clone Wars theatrical release. Jabba orders execution of Anakin/Ahsoka. Interrupted by Padme and Ziro confession.
    2. Duel of the Droids. Grievous almost executes Ahsoka, but she manages to slip from his clutches
    3. Mystery of a Thousand Moons. Ahsoka, among the others, sacrifices herself to keep the deadly virus of Dr. Vindi contained. Saved by an antidote root found by Anakin/Obi-Wan
    4. Cargo of Doom. Sucked into space through an airlock opened by Cad Bane. Pulled back by Anakin (sacrifice + execution actually. Ahsoka refuses to exchange her life for opening the holocron)
    5. Weapons Factory. Self-sacrifice by exploding the factory reactor. Saved from debris by Anakin.
    6. The Academy. PM Almec orders execution of Satine, cadets and Ahsoka. Saved herself.
    7. Assassin. Wounded by Aurra Sing when Ahsoka shielded Padme, saved by the latter
    8. Overlords. Ahsoka is one of the options for possible execution to be chosen from by Anakin.
    9. Altar of Mortis. Sacrificed by Brother, resurrected by Daughter
    10. Padawan Lost. Captured by Trandoshans for a hunt (slow execution)
    11. A Friend in Need. Death Watch (Vizsla) commences the execution procedure of strained Ahsoka, but she is helped out by R2
    12. The Wrong Jedi. Should be sentenced to execution by the Republic Tribunal, saved by Anakin
    13. Ahsoka Novel. Execution procedure by clones after Order 66, probably warned in the last second by Captain Rex, who as a clone also heard the command.
    14. Twilight of the Apprentice. Ahsoka chooses to sacrifice herself in a futile attempt to save Anakin from Darth Vader
Footnote: given Ahsoka was the best padawan during the Clone Wars, the mortality ratio among other padawans should be unimaginably high.
Spoilers: The Rebels finale. Ahsoka will be sentenced to execution and enjoy self-sacrifice. Lady Tano definitely has a romance with death. A convor thinks she is still standing in the point of return. Curious.

Interesting observation. I agree, the mortality rate of padawans during TCW was probably incredibly high. So was the mortality rate for Jedi in general though. I do not think this is foreshadowing Ahsoka being executed / sacrificed at the end of Rebels. I do not think she will be executed at all, if she dies she already tried sacrificing herself (I do not think she will do it again especially if there is no reason to do so.)

In the Clone Wars, Obi-Wan evolved a tendency of being beaten by numerous adversaries, Anakin had a talent of being captured and released. Never miss an opportunity of interrupted execution/sacrifice was a distinct Ahsoka's trait. But yes, there is no substantiated need for that in The Rebels finale, only personal lifestyles that meddle into necessities.

I do hope Ahsoka can overcome that one way or another

I'd imagine if Rebels is going to show Ahsoka's death it'll be more of a sacrifice than an execution. You'd think the Empire would have more pressing matters to deal with than to stop what they're doing and set up a trial and execution of a rather unimportant ex-Jedi at this point in the game.

The real reason for why a self-sacrifice would be better is because it's much more suitable for the character from a storytelling perspective. A character chooses self-sacrifice, they don't choose an execution (unless they could escape it but chose not to for some reason in which case in falls under the self-sacrifice category). One of the differences between heroes/main characters and secondary characters is that the heroes are masters of their own fates so if a storyteller has a choice between self-sacrifice or execution for a hero they should choose the former. Unless they're bad at their job of course.

In Twilight of the Apprentice, Ahsoka's motivation may have been to see if she could save Anakin from Vader of course; but along the way she ultimately allowed for Ezra and the others to escape what was about to happen.
