Jedi Quest 6 explained Yaddle's death. She saves a planet from a toxic bomb and ends up disappearing cause she is vaporized.

 

Koth and Gallia were in the movie sitting on the Council. Just as Stass Allie was at Geonosis. BEHOLD.
