I still think it would've been hilarious if Luke removed the helmet only to reveal that Vader had no head. It'd be like something out of David Lynch.
I got it from this page - http://www.theforce.net/swtc/injuries.html#head - but it doesn't say where it's from, and I can't remember off the top of my head. I seem to recall Lt.Cmdr.Thrawn or TOSCHESTATION posting it a while back. And if David Lynch hadn't passed on ROTJ, it could have happened.
I thought it was a Star Wars Galaxy card, but it might be from Star Wars Chronicles... ? Anybody have it?

I was 11 at the time. Two discussions about the new film ROTJ floated around my school: Jabba the Hutt, how badass Jabba the Hutt was, how badass Lucas was for making Jabba the Hutt from CGI, Leia in the bikini next to Jabba the Hutt. And "you get to see Darth Vader with his mask off!" So I was expecting that. But I was not at all prepared for how it happened. And because of how it happened, I didn't notice his appearance as much--I saw that he was pale and scarred, and that's it. Beyond that I saw a father's eyes meeting his son's for the first time, on his own request, and felt sad for the 20 years they had lost. And I heard "You were right about me...tell your sister...you were right."
I think the whole idea was to see a more or less harmless-looking older man under the fearsome mask. It hits home exactly how human Vader really was. "More machine than man" is one view voiced in the movies, but at the unmasking I think it's the exact opposite. Again, full circle.
Oh Lordy! He needs some bactine or a band-aid or something. I have never seen that before. That is all sorts of messed-up, but in some ways very very cool.
Oh god I love Polish film posters. They make absolutely no goddamned sense, but they're awesome all the same.
I don't remember my first time, but I remember one time during one of my viewings of Return of the Jedi (sometime before Revenge of the Sith came out) the Anakin face reveal struck me with an extra dose of sadness because, as it happened, I couldn't help but flash back to the good-hearted child Anakin of The Phantom Menace the entire time.
German film posters are great too. I love the one for Bullitt. Makes absolutely no damn sense and is terrifying but also hilarious.

A quote from Irvin Kershner in the Annotated Screenplays: Apparently the second draft of ESB suggested he was a mutant, having been hurled down a nuclear reactor shaft, rather than falling into a volcano.
