Take the first 3 letters of your last name.
Take the first 2 letters of your first name.

That is your jedi first name.

Take the first 2 letters of your mother's maiden name.
Take the first 3 letters of your birthplace's city.

That is your jedi last name.

For example,
John Smith, born in Phoenix, Arizona, mom's maiden name is Roberts; his jedi name would be:

Smijo Ropho
