Lightsaber Forms names question

I have a question about the names that the various lightsaber forms have been refered to. In the last Insider, they listed the following names:

Form I - Shii-Cho
Form II - Makashi
Form III - Soresu
Form IV - Ataru
Form V - Shien
Form VI - Niman
Form VII - Juyo

Now there's been some confusion over these names. Mace Windu referred to Form VII as Vaapad, and in the recent Hero's Guide chat, J. D. Wilker officially confirmed this. In the recent post at the WotC site dealing with the Living Force campaign, there were references to From I as Shii-Cho and Form V as Djem So. Now with the new web enhancement for Hero's Guide, we have Sokan, which might be the style Yoda used in AOTC, the reverse-hilt style of Adi Gallia and Nikkos Tyris called Shien, and the two lightsaber style used by Micah Giiett called Niman. I'd like to think Anakin used Niman while fighting against Dooku in AOTC, but it was mentioned in Insider 62 that Anakin used the two-saber variant of Form IV.

So I have these questions. First, for the sake of an argument, I'm going to treat the additional lightsaber variants in the recent web enhancement as additional forms of lightsaber combat. So there are ten forms. Am I right about the following names?

Form I - Shii-Cho
Form II - Makashi
Form III - Soresu
Form IV - Ataru
Form V - Djem So
Form VI -
Form VII - Vaapad
Form VIII - Sokan
Form IX - Shien (hilt held in reverse)
Form X - Niman (two blades)

Next, does anyone know the name for Form VI, since Shien's been taken for Adi Gallia's and Nikkos Tyris' fighting style?

Third, did Yoda use Ataru or Sokan against Dooku?

Fourth, did Anakin use Ataru of Niman against Dooku when he had two lightsabers.

Finally, when Sora Bulq fought against Mace in Jedi - Mace Windu, did Sora Bulq use Vaapad, as he and Mace said he did, or did he use a combination of Vaapad and Niman, since he used two lightsabers during that fight?

I'm guessing still Vaapad, considering I believe Mace also uses two lightsabers in Shatterpoint (yes, it's a minor spoiler).

I don't recall if that's 100% accurate though.
