There is also the incriminating line by Kenobi in ROTJ:

Obi-Wan: Your father… was seduced by the Dark Side of the Force. He ceased to be Anakin Skywalker and “became” Darth Vader. When that happened, the good man who was your father was destroyed. So what I told you was true… from a certain point of view.

Obi-Wan: Luke, you’re going to find that many of the truths we cling to depend greatly on our own point of view. Anakin was a good friend. When I first met him, your father was already a great pilot. But I was amazed how strongly the Force was with him. I took it upon myself to train him as a Jedi. I thought that I could instruct him just as well as Yoda. I was wrong.
Luke: There is still good in him.
Obi-Wan: He’s more machine now than man. His mind is twisted and evil.

Kenobi does not seem inclined to believe in Jedi Prodigals.

Well yeah, Ben Kenobi in ROTJ thinks that Anakin can't be redeemed, because he's tried it before and failed. Or at least, that's what should have happened. It would have made the battle on Mustafar and Anakin's final transformation into Vader much more tragic and the movie much better if we see that he almost turns back to the light side, but doesn't. And also you know, it should have happened for consistency's sake, as the OP said.

Lucas didn't want to structure the movie that way. I personally don't see how Revenge of the Sith was somehow in need of even more tragedy. It's a pretty tragic movie.

I think that tragic, what-might-have-been aspect you allude to was captured as well as it ever could have been in these moments here:
